By accessing or using this website you agree to these Terms of Service. If you do not agree, please do not use the site.

Services overview

This website provides information about Headwaters Longevity, a medical practice in Jacksonville and Jacksonville Beach, Florida, and lets you ask us to call you about becoming a patient.

Nothing on this site is medical advice, and asking for a consult does not make you a patient or create a physician and patient relationship. That relationship begins only if and when Dr. Nields accepts you into the practice and you complete the practice’s own intake. Submitting a form here does not confirm an appointment; someone from the practice will contact you to arrange one.

SMS program description

Text messages are optional and are sent by TODO legal entity name pending. There are two separate programs and you may opt in to either, both, or neither:

• Transactional messages. Appointment reminders, scheduling confirmations.
• Marketing messages. Promotional messages and service announcements.

Each program requires its own separate opt-in. The checkboxes are unchecked by default and neither is required in order to submit a form.

Text messaging terms

• Message frequency varies.
• Message and data rates may apply.
• Reply STOP at any time to opt out of text messages.
• Reply HELP for help, or contact us using the details below.
• Consent to receive text messages is not required in order to receive care or to ask for a consult.
• We do not share or sell mobile phone numbers or SMS opt-in data to third parties.

Carrier liability disclaimer

Wireless carriers, including T-Mobile, AT&T, and Verizon, are not liable for delayed or undelivered messages. TODO legal entity name pending is not responsible for messages that are delayed, undelivered, or misdirected due to carrier issues, network problems, or the configuration of your device.

Age restriction

You must be at least 18 years old to use this website and to opt in to receive text messages from us.

Use of website

You agree to use this site only for lawful purposes, and not to interfere with its operation, attempt to gain unauthorised access to it, or submit information that is not yours to submit.

Please do not send us health information through this website. The forms here do not ask for it and it is not the right place to share it.

We work to keep the information on this site accurate, but we do not guarantee it. Opening dates, availability and membership details are estimates and may change. Articles in our Learn section are general education and are not advice about your situation.
