Who we are

TODO legal entity name pending, operating as Headwaters Longevity, is a medical practice in Jacksonville and Jacksonville Beach, Florida. This policy explains how we handle information submitted through this website.

Information we collect

When you ask for a consult, whether through the card at the top of the homepage or the fuller form on the contact page, we collect:

• Your name
• Your email address
• Your phone number
• Your city and state, if you give them
• Which level of care you said you were interested in, if you said
• Your timeline, if you gave one
• Anything you wrote in the scheduling notes
• The date and time of your request, and the page you sent it from

Every form also asks, separately and optionally, whether you consent to transactional text messages, to marketing text messages, to both, or to neither. We record exactly what you chose, along with the date and the page you chose it on.

We also collect limited technical information automatically, such as your browser type, device type, referring page, and general location derived from your IP address, through website analytics.

We do not ask for and do not want health information here. There is deliberately no symptom, condition or medication field anywhere on this site, and the scheduling notes box asks you not to include medical details. Please do not send us details of a medical condition, a medication or a symptom through this website. If we need clinical detail we will collect it after your consult, inside the practice’s own systems, never here.

How we use your information

• To call or email you back about the consult you asked for
• To answer a question you sent us
• To arrange and confirm appointments
• To send text messages of the type you specifically consented to, if you consented
• To understand how people find and use this website, in aggregate

We do not use anything you tell us about your health to target advertising to you, and we do not pass it to advertising platforms. This is why the forms on this site do not ask for it in the first place.

Text messaging (SMS)

Text messages are entirely optional and are never a condition of receiving care. Every form has two separate checkboxes, one for transactional messages and one for marketing messages. Both are unchecked by default, neither is required to submit the form, and submitting is never gated on either. Asking for a consult by phone and email alone is always available.

• Transactional messages include appointment reminders, and scheduling confirmations.
• Marketing messages include promotional and service announcements.
• Message frequency varies. Message and data rates may apply.
• Reply STOP at any time to opt out. Reply HELP for help.
• You must be 18 or older to opt in to text messages.

We do not share or sell mobile phone numbers or SMS opt-in data to any third party, and we do not share it with affiliates or marketing partners under any circumstances. Consent to receive text messages from us is not shared with anyone.

Cookies and tracking technologies

We use essential cookies required for the site to function, and analytics cookies that help us understand site traffic in aggregate. When advertising is active we may also use conversion tracking to measure which campaigns lead to consult requests.

Most browsers let you refuse or delete cookies through their settings. Refusing non-essential cookies does not prevent you from using this site.

Data security

This site is served over HTTPS, and information you submit is transmitted using TLS encryption. We store it with reputable service providers and limit access to people who need it to contact you. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.

How we share your information

We do not sell your information. We share it only with service providers who help us operate this site and contact you, such as our website host and our customer relationship management system, and only to the extent they need it to perform that work. We may disclose information where required by law or to protect our legal rights.

As stated above, mobile phone numbers and SMS opt-in data are never shared or sold to third parties for their own marketing.

Data retention

We keep your information for as long as needed to contact you about the practice and to meet our legal and recordkeeping obligations, including records of SMS consent. You can ask us to delete it at any time.

Your rights

You can ask us to show you what information we hold about you, correct it, or delete it. You can unsubscribe from email at any time using the link in any message we send, and you can stop text messages at any time by replying STOP. To make any of these requests, contact us using the details below.

Changes to this policy

If we change this policy we will post the updated version on this page with a new effective date.
